Transition planning is an important step in identifying needs, developing goals, and making connections with supporters within and outside of the public school system. The transition planning process begins before your child’s 14th birthday and continues through graduation from high school. We will discuss student transition in each annual ARD meeting as it is a “working plan”, meaning it will change as you and your child’s goals for their future change.
The transition planning process is designed to be results-oriented, and focuses on improving the academic and functional achievement of the child with a disability. Our goal is to facilitate the child's movement from school to post-school activities, including training, education, employment, independent living, strengths/weaknesses, interests/preferences and community participation.
